    The treatment plan

For this patient in her 70s, I combined a deep plane neck lift with bilateral upper and lower blepharoplasty and facial fat grafting. I looked at her neck contour, eyelid heaviness and facial hollowing together. These photographs show her results with Dr. Suzanne A. Trott in Beverly Hills.

    The jawline and neck

Look at the contour beneath her chin and along her neck. It is smoother, and the jawline is more defined. Her deep plane neck lift addressed the deeper neck structures rather than relying on skin tightening alone.

    The eyelids

For both upper eyelids, I performed upper blepharoplasty with nasal fat pad removal. For both lower eyelids, I used transconjunctival lower-eyelid fat transposition. That means the incision is inside the lower eyelid, allowing me to reposition her existing eyelid fat into the hollow beneath the eye.

    Facial fat grafting

I also placed facial fat grafting in her nasolabial folds, the folds beside the nose, her marionette lines below the corners of the mouth, and her lower eyelids. This is different from fat transposition. Transposition moves existing eyelid fat; grafting adds transferred fat to the areas we are treating.

    The overall result

What stands out is the combination: a smoother neck and a more rested eye area, while her expression remains recognizable. In her 70s, the aim was not to give her a different face. It was to address the specific changes that were making her look tired.
